Population Health Nurse

FTE:80 hours bi-weekly
Location:Houghton or Marquette County Clinic
  

Summary:

We are seeking a Population Health Nurse to help to coordinate clinical quality improvement and population health initiatives of all service areas, providing guidance in population health, workflow support, and all other clinical quality-related activities.  This position will require weekly travel to other UGL clinics, but the primary work office will be in Houghton or Marquette County.

Responsibilities  

  • Works alongside clinical staff to review and improve current clinical workflows, conduct performance assessments, and recommend/implement changes and improvements. 
  • Coordinates and executes care gap outreach.
  • Provides patient education.
  • Assists clinical nursing staff with outstanding tests and referrals.

Job Qualifications:

Qualifications 

  • Current MI licensure as an LPN or RN required.
  • Bachelor’s degree in healthcare-related field or 5 years’ experience in a primary care or healthcare quality setting preferred.
  • Experience with PCMH preferred.
