Summary:

We are seeking a Preventive Health Navigator in Houghton or Marquette County who will be responsible for connecting patients to preventive care services through outreach and education. This position focuses on promoting annual exams and preventive screenings, patient outreach, and communication with insurance companies.  The role ensures that patients are informed about and have access to preventive care services to improve overall health outcomes.

Primary Responsibilities

  • Conducting outreach to patients to encourage participation in annual health screenings and assisting in scheduling.
  • Ensuring patients have access to services by addressing barriers and verifying insurance coverage.
  • Identifying and participating in new outreach opportunities to connect with underserved or at-risk populations.

Job Qualifications:

Qualifications

  • High school diploma or equivalent, associate’s degree preferred.
  • Experience working in a healthcare environment.
  • Experience with HEDIS standards and Primary Care best practices preferred.
  • Familiarity with preventive screenings preferred.
  • Strong organizational, communication, and problem-solving skills.
